Custom form fields for payment form

What would you like to see? Please describe in detail.

I would like to be able to add a (variable) number of custom form fields to the 
payment form from the settings menu.

I would like to have a checkbox to make these fields hidden.

Test cases.

The form fields will show up with default values saved in settings panel when 
viewing the source page.

Please provide any additional information below.

Some advanced integration of Moneybookers for example would request more fields 
to be submited to the processing page. Unless a developer would build a special 
page template or code with Javascript, having such options in the settings 
panel would be very handy.

hypothetical one.

I think something like this: the option panel in figure 63a allow the admin to 
add a new custom form field (the example is real case) and the result is 
depicted in figure 63b
