Fix : #31

Contributions :

Fixed the urls in frontend going to wrong urls in DiseasePred Added 3 react components for DiseaseReport , HeartDiseaseForm and KidneyDiseaseForm Connected the Components to the Remote Backend Replaced the NA values in Kidney and Heart Disease Report with actual normal values
