Delivery Address and Gift Address #61

The order form for a package as a gift has both form fields for

delivery address
gift address

To make the order process clearer only the gift address should be shown.

This change affects

the order form
to order overview
the confirmation email

A new image needs to be uploaded (to assets/packets/gift_100x100.png). The image will be displayed in the top right corner of the order overview when ordering a packet as a gift:
