Minor changes to module structure and shadowban fixes #756

I changed the module structure slightly:

Renamed EmptyModule into IncompleteModule for clarity on what the module does
Moved ThumbnailModule into its own package, so that LimitingInputStream is now together with the module it belongs to.

Some minor fixes for the new shadowban module:

Don't throw errors when bug report is already resolved
Execute as the very first module so that other modules are less likely to interfere
