chore: updated GitHub templates to use forms

What are the main changes you did:

• Updated the GitHub templates to use GitHub forms.
  • While it is a public preview, the public beta started back in 2021. So might be worth trying if initial test in live demo seem fine?
  • They are confident enough to suggest people to use the forms, to cite:

    [...] We recommend using the upgraded multiple issue template builder or issue forms to create issue templates.

• Some adjustments to the fields (f.e. merged the screenshot section into the description to reduce number of fields).
