Fix: changes to the clinical table

In the table cosas_clinical, there are few minor fixes and changes. These are listed below.

[x] Change label for attribute excludedPhenotypeHpo to Excluded HPO
[x] Fix label for observedPhenotype (missing the "e")
[x] Change displayed value for provisionalPhenotype and excludedPhenotype; make sure the description is displayed (related #3)

This issue is dependent on #3, but it is stuck at the moment. I am in the process of creating a HPO workflow.

For now, I will remove codes that do not exist in the reference dataset.
