This PR:

Add namespace fwdpp::debug, which is based on exceptions rather than assert.
Replaces existing assert calls with exception handling wrapped in preprocessor code.

The result will be that -DNDEBUG renders functions in fwdpp::debug empty and that #146 will be resolved.
