MAXO structure: Medical device usage, device settings in treatment

There are some fairly specific recommendations that include a medication + medical device or device settings that are specific for treatment or combining 2 treatments together.
Ex. #158

• nebulizer usage

• MDI Metered-dose inhaler/Albuterol MDI usage,

• ventilator setting recommendations.

• prone positioning while ventilating.

This is just a placeholder to think about whether terms should be added and how the MAXO structure should accommodate it.
