some alternate labels missing for omim #84

we need to make sure that we add the “other entities” (includes) as synonyms, for example: http://omim.org/entry/101600 Pfeiffer syndrome needs to have Craniofacial-skeletal-dermatologic dysplasia, and CRYPTOPHTHALMOS WITH OTHER MALFORMATIONS

@cmungall for the example of Pfeiffer syndrome, would you like me to clean up the labels and strip the "included" from "CRANIOFACIAL-SKELETAL-DERMATOLOGIC DYSPLASIA, INCLUDED" ?
