Update ingests to basic Biolink 3.1.1 compliance and cat-merge behaviour by removing 'source' but avoiding use of 'provided_by' #395

Resolves issue #389: Biolink Model updated to >=3.1.1; removed 'source' references for nodes, converting to 'provided_by' (had already been done elsewhere...)
