GARD: Decide on what to do with the 785 GARD ids which are mapped to obsolete Mondo IDs

Right now, we have 785 GARD identifiers that map to obsolete Mondo IDs. I think we need to

1. Generate a table for all of these with exclusion reasons
2. Review reasons briefly and as a consequence:
   • EITHER argue with GARD team on why these should not be included as diseases
   • OR revive them

The third option, just saying that "GARD" considers them diseases but we don't, is not an option IMO.
