For Hs configuration, add option for diseases and genes #86

We should add an option to add human gene-phenotype associations as well as disease-phenotype associations. Needed for https://github.com/NCATS-Tangerine/ncats-ingest/issues/25

For now, disease-phentoype, patient-phenotype, and gene-phenotype are all indexed when configured for Hs, see aad70b1. Will close although this doesn't implement the original request. Please reopen if it would be useful to make the category configurable.
