Update PR template for the docs consolidation project #3256

This PR updates the PR template to add consolidation-specific checklists for the PR author and reviewer.

I've renamed the existing template to pull_request_template-base.md to preserve it in the repo, so we can easily switch back to it after the consolidation project is complete.
