Allow breadcrumb to be fixed to scroll with page #551

relates to what is discussed here https://tracker.moodle.org/browse/MDL-73505 where when editing as a teacher role across multiple courses it is hard to quickly see which course you are in. I think the course index's context and purpose loses the previous nav elements that used to give more indication.

Just an idea but perhaps it could be possible in boost union settings to allow the breadcrumb, if enabled, to be fixed under the top nav banner and scroll with the course page. This would then at least give a good visual indication of whereabouts.
