Password prompt should specify user/hostname #254

What steps will reproduce the problem?
1.Initiating a connection over MacFUSE using SSHFS. 
2.Letting a connection timeout, then prompt for a reconnect password

What is the expected output? What do you see instead?
Include the hostname and/or user for the connection on the password dialog.
Instead, a nondescript password prompt appears. Since I use multiple
simultaneous connections, determining which one it is prompting me for is a
guessing game. 

What version of the product are you using? On what operating system?
MacFUSE 4.0, SSFHS 3.0

Please provide any additional information below.
The product is simply awesome, this is the only thing I would like to see
changed.
