Make name titles configurable #30

We should add a config file for the available user titles. A reasonable default would be:

Mr.
Mrs.
Miss.
Ms.

This will be used to populate the user title drop-downs all around the system.

Original issue is messagedigital/cog-mothership-cp#111

I'm upvoting this change, it's not great that there is no 'Ms' (or 'Doctor') option when you set up an account. I'm also labelling as a bug as it comes up every UAT.
