Figma designs

This is to implement the modal view with the survey options.

Closed state

Header with
- Firefox icon
- Title
- Close button
Survey content
Footer with
- Submit button (disabled unless option selected)

This will focus on iPhone. Landscape and iPad layout will be addressed in a separate ticket if does not come easily. Keep in mind RTL UI layout and accessibility when working on UI as well.
