Add email-to-download form for Firefox Focus page

Description

When we created the Firefox Focus page, we opted to only provide a QR code because of time. The focus team has requested an option for email-to-download. Lets work with our activations team to get a mailing list set up and an email-to-download option for the Firefox Focus page.

Considerations:

• Should this be a test?
• Should we have both the QR code & the email-to-download option live at the same time? This way the product team can run their tests. We could use URL parameters to determine which one is shown.

• [ ] Provide unique adjust link for iOS & Android
• [ ] Work with activation team to set up list & email
• [ ] Code

My recommendation for now: change this page to use email-to-signup, but keep the QR code at a variation such as ?signup=qr so that we can link and use it when desired.

@craigcook i was mistaken and an ID does not exist yet because this email hasn't been created yet. I put in a request and will update when available. There also may be a small change to how this is handled, so lets put on hold briefly till I can sort out

This issue was/is blocked because the relevant sign-up email hadn't been created in our mailing systems.

However, given the age of the ticket, we should check: @craigcook @slightlyoffbeat Do we still want an email-do-download option for Focus or can we close this issue as unplanned?
