write documentation covering upgrading from ES 0.90 and earlier to ES 1.0 #247

This release should include some documentation covering how to upgrade to Elasticsearch 1.0+ with ElasticUtils. Everyone's situation will be different, so this should just be a high-level set of steps to give people a rough guide to work with.
