Design sign-up screen for mobile #31

Not needed for teach. Moving to backlog.

I'm confused, is this b/c we are doing responsive design instead @thisandagain @secretrobotron? This is actually done, so I am going to post designs here and move it forward with a final design review.

Note for @alicoding: with the way the field labels are designed, we will need to add hidden labels for screen-readers, as per @k88hudson. Also, the header bar (currently grey) will need to match whatever UI is designed for for the new tool. cc @xmatthewx

We should be clear on the split between "Desktop" (which means "browsers" and with our audience means primarily desktop users) and "Mobile" (which means "app" on Android). For this SOW we need responsive designs (optimized towards desktop) for browsers.
