Fininsh rendering monitoring dependency relationships in opsview and establish ELB membership monitoring #97

I've implemented this at us-east-1, do so in the other environments.

Also establish monitoring the status of an instance as healthy and live in an ELB. Set this status as a dependent monitor for all other monitors on the host.

Setup a monitor to look for minimum percent and minimum number of helathy hosts in an ELB
