[STR]: 1.Log in https://mresponse-staging.herokuapp.com/ 2.Go to Respond section 3.Add a response and click submit button

[Actual result]: Console errors:https://drive.google.com/file/d/1HnZXkaM4BgXqJ0B9D0yzKsL2OmzdYnR2/view?usp=sharing The UI looks good and the response is displayed in the admin interface
