Form design #93

Based on old design ticket https://github.com/MozillaFoundation/Design/issues/165 and examples in https://github.com/mozilla/mofo-bootstrap/issues/75

Should be based on Bootstrap http://getbootstrap.com/css/#forms (does Boostrap 4 have forms?).

We can use the form style that is being designed for in Pulse v2: https://redpen.io/nvd9c7a75f06c79e35 and https://redpen.io/ov0d7f8ba9c437ee56

@mmmavis would you be up for us 'perfecting' the form styles in Pulse and then when we're happy with then, add it to mofo boostrap?

I think mostly how we handle form errors.
