Add Click-able Index to top of pages with lots of info in them #18

I have received this request many times... It would be very helpful if the following pages had a click-able index at the top of them (for the main section headings): https://ccadb.org/policy https://ccadb.org/cas/fields https://ccadb.org/cas/intermediates https://ccadb.org/cas/updates
