[x] Code of conduct
[x] Revised changelog

PR template should include directions for authors to check that they've linked in their new feature to the docs correctly, and that they've checked that it renders correctly.

Need an issue template for tagging a release that includes things like checking the changelog, docs, etc.

The changelog should be updated to follow Keep A Changelog semantics.

For pull request template, also add that they have tested GPU code locally if they added any

We don't get enough PRs / bug reports to need these, so closing for now. Someone can reopen and implement if they want them.
