Selection of PMPI functions is listed in the function index #818

Problem

A selection of PMPI functions are listed in the function index due to their typesetting as MPI functions in the changelog and semantics chapter.

It is unclear for users why only those are in the function index and others are not.

Proposal

There are two different ways of pursuing this: a) Keep those functions listed in the functions index because the special case of changing semantics makes them sepecial enough, or b) Remove those references from the function index.

Closed this as 'won't fix' as we explicitly want to keep the references to PMPI functions no longer allowed as macros.
