Next friend e-signatures #68

Related to #67, support needs to be added for a next friend's e-signature.

If the user has a next friend and said friend agrees to e-sign, THEIR name should appear after the /s/ in the e-signatures. Additionally, their name should appear in the "Name (type or print)" fields in the respective signature blocks.

Note that the A2J interview asks for the next friend's e-signature consent toward the beginning of the interview, right after asking for the next friend's name. Because of this, the e-signature page in the outro questions can be skipped if a next friend is involved.
