GHA #53

Adds GHA to:

  1. Perform CI (Mac, Windows, Linux) for all branches
  2. Auto render the Readme Rmarkdown for master
  3. Codecoverage

This required me to disable the Appveyor link - so any builds using that will now fail I think.

FYI the travis and appveyor checks don't appear on the checks in branch (just in this PR). I think when merged it should get rid of them.
