Realoading VSCode required for the plugin to detect Hg and changes in working directory

The extension does not detect changes when the project is first opened in VS Code. Always have to reload VS Code for the extension to work.

Extension version: 1.3.0 VS Code version: 1.37.1 Mac OS version: 10.14.6 (18G87) Hg version: 5.0.1
