Resolve use of deprecated PublishPress Authors `inline_avatar` layout #21

I need to dig into this more, but the old PublishPress Authors layouts have somehow been deprecated. I now need to enable the legacy inline_avatar in the settings to make it work. I need to upgrade this so it works without a settings change.
