Request VM page enhancement #19

Following enhancements are required for request VM page:

  1. There should be a add/verify button for collaborators and faculty approval fields. User after typing the faculty or collaborator field have to click on that button to verify if username is correct or not. If incorrect an error should be displayed for incorrect user name of faculty or collaborator.
