The feature:

When selecting to create an ADR, allow the user to select a specific ADR form

So, rather than a single, generic ADR form as we currently have, have many different ADR forms that a user can select from

I think there might be something here also to do with specific forms for specific vaccines/items 🤔

@mark-prins this has come up - can wait till after deployment to implement but wanted to have a plan in case it made big waves
