CSV Import: Offer to create payment methods #872

If a column in CSV Import is mapped to Payment Method and has data that does not exist yet as payment method, there should be an option to create new methods.

I had assumed that existing methods would already be recognized, but surprisingly, this had never been implemented: #1584
