I think we should use the tabular format for only the steps part. 

You should also enter
Module Name
Description
Test Designed By
Test Design Date
Test Executed By
Test Execution Date
fields. 

In the steps, there should be fields for (to be filled later) Actual 
Result, Status (Pass/Fail), and Notes

In the test case 5, the user should add content (into a text box) for 
contribution, not only tags. And as a result, the content will be shown on the 
case page. 

You should also specify a field for test data to be entered (like into a 
textbox, or menu item, etc..)

Rating should be done in terms of accuracy and efficiency separately.

I will not change anything related to rating scale ( accuracy, efficiency) to 
provide consistency between test case and use case.
