Allow "universal" link to form #312

The form link for a given patient on a given form is not something that can be shared as it is tied to the patient

Allow urls like 

SITEURL/actions?action=form&form=AngelmanRegistryBehaviourAndDevelopment&registry=ang

This only allows the user to browse of they are:

The patient themselves ( registries like MTM) The parent guardian and the supplied id is one of their children or no id is supplied and the first child's record is browsed The user is in the same working group(s) of the patient with that id
