SSHFS should name the host and username when prompting for a password

What steps will reproduce the problem?
1. Mount several remote hosts
2. Periodically see SSHFS prompt you for a password for one of them,
without specifying which
3. Try every password you can think of, get frustrated at repeated questions

What version of the product are you using? On what operating system?
MacFUSE 0.30

Please provide any additional information below.
The GUI password prompt should specify both remote hostname and username,
given that you may be logged into the same host several times as different
users.
