Less efficient Give Feedback Option

i would like to add some images to make it more beautiful and readable by adding some features . i would also add phone no option in the give feedback one! please assign me with required labels with Gssoc extd'2024 program

@BVPKARTHIKEYA Adding small icons like person silhouette for name, envelope for email, etc is a great idea. But I don't think we need the phone number of the reviewers. I'm assigning it to @BVPKARTHIKEYA. Don't forget to create a new branch gssoc-yourName before sending the PR.
