added support for pod annotations #7

I added support for pod annotations.

This is useful when using things like fluentd-elasticsearch, pod annotations can be retrieved by fluentd to be sent along with pod logs.

@mumoshu i took the opportunity to upgrade the prometheus-process-exporter image to 0.5.0 also

👍 i forgot to update the app version in the chart.yaml file, it should be 0.5.0, not 0.4.0. thanks
