various minor corrections to chapter 12 #158

there wasn't a whole lot to do with this chapter, so this is pretty straighforward. This does not consider #38 yet, even though many examples were touched. However, #38 should be done in one go for the full guidelines, so only after all chapters have been addressed… This one fixes #91

Again, I believe it's sufficient if one reviewer checks if everything compiles properly…

I just checked once more if things work properly, and they do. I'm sure we can sort out remaining issues after having pulled this…
