StructureMap Exception - No Default Instance defined for PluginFamily #234

Frequently I get this error in one of my apps:

StructureMap Exception Code: 202 
No Default Instance defined for PluginFamily RequestReduce.Configuration.IRRConfiguration, RequestReduce, Version=1.8.76.0,     Culture=neutral, PublicKeyToken=null

What could be happening?
