Duplicate error "fallback item could not be found" #276

PROBLEM:

Validating EPUB 3 testcase "expanded/invalid/fallbacks/nonresolving" reports 
the error "fallback item could not be found" twice

RESULT:

Epubcheck Version 3.0.1

Validating against EPUB version 3.0
ERROR: fallbacks-nonresolving.epub/EPUB/lorem.opf(14,82): assertion failed: 
manifest item element fallback attribute must resolve to another manifest item 
(given reference was 'img4')
ERROR: fallbacks-nonresolving.epub/EPUB/lorem.opf(14,82): fallback item could 
not be found
ERROR: fallbacks-nonresolving.epub/EPUB/lorem.opf(14,82): fallback item could 
not be found
ERROR: fallbacks-nonresolving.epub/EPUB/lorem.xhtml(21,33): non-standard image 
resource 'EPUB/image.xyz' of type 'image/xyz'

Check finished with warnings or errors

Seems to be related to issue 239 - maybe I had overseen something when fixing 
239...
