When I check the attached epub file with epubcheck (via web) I obtain the next 
error message:

elements from namespace "http://www.w3.org/1998/Math/MathML" are not allowed

Readers like Calibre, Readium or IBooks do not have any problem to display the 
file correctly.

MathML is allowed in EPUB 3, your sample book is EPUB 2.01.

Filing this issue as invalid.

Yes, EpubCheck detects the version automatically. On the web valdator it 
correctly says "Detected version: EPUB 2.0" when validating your sample EPUB.

If you want to make your EPUB a valid 3.0 publication, please refer to the EPUB 
3.0 set of specifications:
http://www.idpf.org/epub/30/spec/epub30-overview.html
