Create "My Sleep Apnea Education" survey for diagnosed users #873

Hello-

There were a few questions that were overlooked in our final deployment of the adult surveys that were in the original matrix of questions that Tina had pulled together. In talking with Kyle- it seems like the best route for inclusion would be to create a new survey titled, “My Sleep Apnea Education”. I think we could add to this battery since it is pretty short. Susan- I was going to check with Jessie to see if there are other education related questions that we should think about including. Thoughts? These are from Dr. Parthasarathy’s questionnaire.

Did you receive adequate education about sleep apnea from your providers? Did such education improve your understanding of sleep apnea Did such education help you realize the risks associated with sleep apnea

If you did not receive satisfactory education regarding the risks of sleep apnea from your physician and sleep center, did you receive it elsewhere (if yes, where)

Looking back on this, this seems to have been covered largely in the "My Sleep Apnea Information Satisfaction" survey released a couple months ago. Closing this particular issue.
