Allow multiple Studies to have their own Consent and Surveys #955

The MyApnea surveys will be grouped under the new MyApnea study, with the child surveys being grouped under an individual study (MyApnea Child) or similar.

Studies will require characteristics of the user and can be limited based on the studies inclusion/exclusion criteria.
