Place for GNU Radio plug-in to be placed into (submodule or straight copy)
lime::SDRDevice::Get/SetNCOIndex()
Getting the phase offset from lime::SDRDevice::GetNCOFrequency()
Missing connection in XTRX

Changed

Moved plug-ins into their own folder for less clutter
Moved Logger and VersionInfo includes into the public includes folder
