Send surveys when a problem is closed, rather than after a specific time. #1240

@Lynne2 as part of this I had to change some of the email and confirmation text, so that it matches the new situation (because previously it told people about the survey interval and asked if their problem was closed, as I mentioned earlier).

Here's the new text, can you confirm if this is ok?

On the confirmation page:
