This works well on http://alaveteli.org/deployments/ and would be equally beneficial on the FMS.org site. (Also wouldn't hurt on the mySociety.org site)

This exists at http://fixmystreet.org/sites/ but I'm not sure it's linked from anywhere?

The homepage seems like an obvious place to link from - there's even a nice blank space under the intro paragraph.
