Missing bits for FMS Oxfordshire #515

Following on from #502 we need to pick up the following:

[x] make the problem reference no. more obvious
[x] reduce the amount of text in the report a problem form
[x] link 'what is an emergency problem' through to the #pothole part of help page
[x] Remove "Click on a pin and then on the “details” link to see an individual report detail." text from under the map

Changing the display of the reference number is going to be a bit trickier as the logic is currently tied up in a method on the model, so would require some refactoring to get that data into the template.
