Using StreetReport app, not the FixMyStreet app (you can see this on any of his reports). It looks like he submits very close, very similar reports, so I don't think he's getting two questionnaires for the same report. e.g. #387108 and #387109 are what he's presumably referring to. We only sent one questionnaire for each.
"I am submitting my reports via my FIxMy Street App. After 4 weeks I am receiving TWO requests for a questionnaire to be completed. Please can this be corrected? I have been receiving TWO for each problem reported, which seems most odd".
