Double-submitting the account creation form can produce a constraint violation error #626

A ActiveRecord::StatementInvalid occurred in accounts#create:

PGError: ERROR: duplicate key value violates unique constraint "index_users_on_cached_slug";

Fixed in ad2c40e9f8ea6adb579d4f66a977ecebfa7064a7
